CentraCare Home Health provides remote patient monitoring to manage your health at home. The monitor is individually programmed for you according to conditions set by your health care provider.

Remote Patient Monitoring may be ideal if you have:

How Does Remote Patient Monitoring Work?

  • In just three minutes each day, the monitor collects a variety of vital signs such as heart rate, blood pressure, oxygen saturation, body weight and temperature.
  • The monitor also gathers information on your fatigue and general wellness levels by asking a variety of customized questions.
    • For example, if you have high blood pressure, you might be asked, “Are you experiencing any dizziness today?” You respond to each question by pressing the “yes” or “no” button on the monitor.
  • The data collected by the monitor is reviewed by our clinicians, who can find and deal with even the slightest abnormalities before they become serious. This helps prevent emergency room visits and hospitalizations.
