If you have mobility issues, being able to have services delivered to you in your home is a benefit. CentraCare Home Health Services provide expert care and guidance to adult and pediatric patients needing rehabilitation in their homes. Our patient-centered care promotes patient independence. We also provide support and education for the family caregivers.
The focus of rehabilitation is to improve your overall health and safety at home by:

Your primary health care provider oversees home health services and receives frequent updates from our staff on our progress and recovery.
Rehabilitation Services
If you may need Rehabilitation Services if you are recovering from:
- Total joint surgery
- Stroke
- Cardiac issues
- Respiratory diseases
- Complications from a chronic illness
- Genetic or neurological disease
You will receive a home safety assessment with proper recommendations for home adaptive equipment. We also will create an individualized home exercise program is created for you — focusing on reduction of falls and helping you rehabilitate to your full potential.

We will diligently assess and guide you as you recuperate at home.
Referrals/Insurance
We accept self-referrals or a referral may be made by a health health provider, family member or friend. Once the referral is made, our staff will contact your health care provider to discuss the services needed. A nurse or therapist will provide a free initial assessment at home. Most insurers cover skilled home health services. Home health charges will be billed to you and submitted to your insurance company (Medicare, Medicaid, Medial Assistance/County Waiver programs and/or private insurance). No one will be denied services because of an inability to pay.
