The program provides support through in-person and video visits, phone calls and a help line. Some people are also eligible for devices that can monitor health at home, helping to avoid hospital visits.

Experienced Staff
More than 2,500 people throughout Minnesota receive care through our program each year. A dedicated heart failure team provides personalized treatment, taking the time to get to know you so you can set goals and manage your daily living. Our heart failure nurses are certified in heart failure, meaning they have specific knowledge on the topic and passed a certification test.

While the "My Heart" Program provides holistic support for heart failure, BRIDGE ensures you receive the right care and support in the later stages of the disease.
The program helps you set goals and manage symptoms that can affect quality of life, such as depression, stress, fatigue, insomnia and labored breathing.
As part of the program, a specialized clinician will meet one-on-one with you and your family to discuss:
