St. Cloud Hospital Nurse Receives National Nursing Award for Patient Flow
A St. Cloud Hospital nurse has earned a national award for her commitment to making sure patients receive the right care at the right now. Colleen Porwoll, RN, core charge nurse in the Bone & Joint Center at St. Cloud Hospital, is a recipient of the Second Annual DAISY Award for Extraordinary Nurses in Patient Flow.

The award recognizes individual nurses, as well as teams of care providers who are led by a nurse, for their hard work and commitment to ensuring patients have access to the right bed, at the right time, in the right setting.
Porwoll is being awarded for her work in developing a framework for engaging patients and families in the discharge planning process upon admission. Positive results from this new process included the fact that 98% of patients said they felt prepared for their discharge from the hospital.
"Colleen recognized an area of opportunity, researched the literature, and was able to design a discharge model that includes patients and their families in the discharge plan, from the day they are admitted," said Lisa Meyer, director of the Bone & Joint Center at St. Cloud Hospital. "This discharge model improves the communication between the patient and the entire care team, and our metrics have shown that patients feel more prepared to leave the hospital. Colleen's work involving the preparation of patients for discharge goes straight to the heart of our core values: to serve patients above all. She is a wonderful example of going above and beyond, and this recognition is well deserved."
